Care Management Services

 

Our Care Management team members are specially trained to work alongside the physicians and participate in your child’s care. They help identify needs and resources, participate in setting goals to improve your child’s health, assist with medication management, ensure your understanding of the  plan of care and follow up after a visit to the urgent care, emergency room or hospital admission.

Why Participate?

ABC Pediatrics is committed to providing excellent medical care to our patients.  Our care managers have all received certification through the MI Center in Clinical Systems Improvement (MI-CCSI). Our asthma care managers have also been certified through the National Asthma Education Certification Board.  Our care managers can connect with patients utilizing a combination of telephone calls, office visits and virtual visits. 

 

Care managers touch base with patients over the telephone to obtain updates on your child’s health, answer questions, offering advice and guidance.  They collaborate care between patients, parents and the pediatrician. Communication between patient/parents and care managers can often replace the need for an additional office visit with your child’s pediatrician. Telephone calls provide flexibility, mobility and does not require travel or taking your child out of school. Care managers are also able to assist with coordinating care with other specialists, clinicians, durable medical equipment vendors and other entities as needed.

 

The cost of care management services is covered by many insurance companies.  Our billing department would be happy to provide coding information for our patient families interested in contacting their insurance carriers to check on policy benefits.
